In the springtime my colleague, Dorothy Fried, LCSW, CMC and a well-respected Geriatric Care Manager (GCM) in the healthcare community contacted me. A professional Geriatric Care Manager (GCM) is a health and human services specialist, who helps families that are caring for older relatives. A GCM is trained and experienced in any of several fields related to care management, including nursing, gerontology, social work or psychology.

While assisting on this year’s GCM planning committee for their annual National Association of Geriatric Care Managers (NAGCM) Western Conference, Dorothy nominated me to be invited to submit a proposal on “Fall Risk” their 2014 annual conference.

A few months later, I received word that I was chosen. I felt honored.

Putting the presentation together was not an easy task. I have worked diligently on my Power Point, forever tweaking the information so it is clear, concise and fun. Without the help of my assistant, Sara, friends, nurses and GCM’s, who supported my efforts 150%, submitting my information and developing the presentation would have taken me twice as long! I am forever grateful!

Experience Working with GCM’s

I regularly work together with GCMs as well as conservators to minimize their client’s age-related decline. I either receive a call to conduct my initial client assessment after the GCM or conservator has completed the evaluation, or when they notice a decline in their client’s health over time. Oftentimes, their client would have completed a round of physical therapy and they, along with the client’s physician, recognize the client would greatly benefit from a formal exercise routine at home (post-rehab), since it’s difficult for their client to leave the house to attend a class at a senior center.

My presentation is called “Keeping Your Client Safe: Physical Activity & Good Nutrition”. You’ll have the opportunity to learn reasons for the rapid decline of our seniors as it relates to balance.
