Physical Activity Questionnaire (PAR-Q)
A Physical Activity Readiness Questionnaire (PAR-Q) is one of several assessment tools required to be completed prior to meeting a new client for their evaluation. We want your physical activity experience to be safe, fun and rewarding. As you plan to become more active, increase your aerobic capacity and strength endurance, Lori asks you answer these preliminary questions. This questionnaire accompanies the ‘customized’ Health Fitness Questionnaire sent when we start planning your program.
- Has your doctor ever said you have a heart condition or high blood pressure?
- Do you feel pain in your chest when you do physical activity?
- In the past month, have you had chest pain when you are not doing physical activity?
- Do you lose your balance because of dizziness or do you ever lose consciousness?
- Do you have a bone or joint problem that could be made worse by a change in your physical activity?
- Is your doctor currently prescribing any drugs for your blood pressure or heart condition?
- Do you know any other reason why you should not perform physical activity?
If you have answered YES to any of the above and do not wish to consult with your physician prior to your first personal training session, you will need to complete a Physicians Approval Letter. Otherwise, we can help you present an informal letter to your physician for a prescription to exercise.
